  4. Emergency Case Reporting Details required for Service Request
    1. Descriptions on Causes to Case Reporting:
      1. Fire occurrence (residence, shop, factory, automobile or weed)
      2. Emergency rescue (traffic accident, acute disease, injury, drunk falling on roads, suicide and mental abnormality)
      3. Public services (bee or snake catching, elevator trap)
      4. Others
    2. Description on case location: Detailed address with room No., and major target nearby
    3. Description of case reporter: Mr. or Miss ×××
    4. Description of case reporter’s phone No.: Information on residential telephone and cell phone number
II.One-stop Window Service
  1. Application and Issuance of Fire Certificate
    Please contact the Fire Investigation Section at 03-5229508-463
    1. Application for fire certificate and objects: Both the property owner of the fire affected households and the current residents (including two or more houses) shall apply for fire certificates. Application can also be made to non-building fire (such as vehicle, ship and crops).
    2. Normal residence application for fire certificate must be attached with the following certificates:
      1. One photocopy of ID card of the applicant.
      2. Personal seal of the applicant.
      3. One photocopy of document for the house property right (for vehicle, a photocopy driving license or relevant certifying documents should be provided; for lease, a photocopy of the lease contract should be provided).
      4. If the application is made by the person who is authorized by the applicant, the photocopies of the Power of Attorney as well as the ID card of the applicant should be attached.
      5. If entirely burnt, a certificate issued by leader of the community is required.
    3. For application made by a company (or factory) of a fire certificate, the following documents must be attached:
      1. One photocopy of ID card of the applicant.
      2. Personal seal of the applicant.
      3. One photocopy of document for the house property right (for vehicle, a photocopy driving license or relevant certifying documents should be attached; for lease, a photocopy of the lease contract should be attached).
      4. For application by a company (factory), a photocopy of the business registration and company (factory) seal should be attached.
      5. IFor pawned articles, a photocopy of the pawn ticket should be attached.
      6. If entirely burnt, a certifi cate issued by leader of the community is required.
      7. I If the application is made by the person who is authorized by the applicant, the photocopies of the Power of Attorney as well as the ID card of the applicant should be attached.
  2. Application and Issuance of Emergency Rescue Certificate

    Documents as required :
    1. A photocopy of front and back pages of the ID cards of the applicant and the injured or patient.
    2. Personal seal of the applicant and the injured or patient.
    3. For authorization, the ID card and personal seal of the authorized person should be carried.

  4. Application and approval of as-built inspection on building fire safety equipment

  5. Application for issuance of certificate for LNG vessel storage room

    1. Application document (if the application is made for the storage location, a copies of application for storage location and the disposal location should be attached)
    2. Photocopies of the front page and reverse page of the ID card of the responsible person
    3. Location sketch and configuration sketch of the storage room
    4. A photocopy of the permit of land use at the business location or documents sufficiently determining the use by zone
    5. An original copy of the permit for LNG storage location (exempt for a new applicant)
    6. A copy of agreement on entrusted storage of LNG vessel
    7. Land use permit for business location or certificate for property rights of houses (if the business location is not owned by the applicant, the Lease Agreement should be attached)
    8. Route certificate
    9. Registration certificate of profit-making undertakings (exempt for a new applicant)
    10. Letter of consent on transfer (attached when the responsible person is changed)
  6. Application for issuance of overhead firework operation permit

    1. Application for overhead firework (filed by the responsible person 5 days in advance)
    2. Photocopy of the ID card of the responsible person
    3. Photocopy of manufacture or import register or filing certificate
    4. Discharge list: The list shall record the description, quantity and specification of the firework to be discharged
    5. Permit for import or peddling
    6. Plan of discharge location marked with safety distance
    7. Specifications on operation location including overhead firework performance, discharge method, discharge facility and attached pictures or photos
    8. Discharge safety protection plan: The plan should be recorded with the discharge time, security, fire protection, emergency rescue, site traffic control, evacuation and other emergency measures.
    9. Photocopy of discharge personnel name-list and specialty certificates
    10. Other documents recognized by a competent authority of municipality directly under the central government, counties or cities
